Medication reconciliation 

Is the process where medications prescribed by a medical practitioner are checked to ensure they match the current list of medications the patient is taking.

 

When does this need to happen?

Most require it whenever a patient is admitted, transferred and discharged or a patient has stopped taking a medication.

 

The process of medication reconciliation?

A list of the patient’s current medications; along with a list of the medications currently needed; these are then compared and a new list is formed which is then communicated to the patients and caregivers.

Intially it's about creating the most accurate list possible of all medications a patient is taking — including drug name, dosage, frequency, and route — and comparing that list against the physician’s admission, transfer, and/or discharge orders, with the goal of providing correct medications to the patient at all transition points within the hospital.
