This service is for patients in the Shoreditch and City PCN deemed as vulnerable. That is: over 75 or living alone and at risk of mobility issues or falls or has a learning disability or visually impaired or obese. High Risk diabetes is excluded.

The Lawson Practice – Friday, 9am-12pm. 2nd Friday of every month (room 3)

De Beauvoir Surgery – Friday, 2-5pm. 2nd Friday of every month.

The Neaman Practice – Wednesday, 2-5pm. Last Wednesday of every month
