Chronic Disease Management

Annual Review of Chronic Diseases

The Neaman Practice is committed to improving care for our patients. As part of this we recall patients with chronic diseases.

The idea is that you will be called once a year, at which time we will organise all the appropriate tests and monitoring of your long term condition. The clinics are run by our practice nurses – Wendy Beckles and Rebecca Darkwa-Marfo. This will hopefully mean you will have better monitoring of your health condition and will be able to have your chronic diseases reviewed all at once rather than making multiple appointments.

Of course you are still welcome to book an appointment with a doctor or Nurse at any time, for any other issues.

We feel this service will benefit our patients and would encourage you to give it a try. Please ring the practice to book your appointment.
